The focus of community mental health nursing to people with dementia has always been problematic. The underlying issue is, who should receive Nursing care? Traditionally, within hospitals, it was the person with dementia who was considered the main recipient of care. While recognizing the centrality of the person with dementia, mental health nursing or, as it was then called, psychiatric nursing, often positioned visiting relatives as marginal
and at worst, an intrusion to nursing care. However, nurses working within innovative community mental health nursing services to older people realized that this understanding‘ of relatives’ was not appropriate in community and domiciliary-based settings and that relatives should be seen as joint recipients of care alongside the person with dementia.
Returning to the key issue of who should receive community mental health nursing care, the approach adopted by innovatory community mental health nursing services suggested that the person with dementia and their carer should be the joint recipients of community mental health nursing care.
Thus, rather than conceptualizing mental health nursing to people with dementia in dyadic terms – the person with dementia and their carer – it became common to see community mental health nursing practice. We would suggest the term ‘dementia care triads’ can be adopted to describe these relationships. We should, however, point out that there may be more than one informal carer involved in the provision of community mental
health nursing to people with dementia and that there are also likely to be other dementia 90 Models of community support and practice values care workers involved.
